From a near miss to verified corrective action
An event that caused no injury can still reveal a serious hazard. Reporting, immediate protection, a lasting correction and subsequent review are different steps.
Illustrative site situation
Illustrative situation: a small object is found under facade work. No one is injured, but location, time, hazard, immediate precautions and later correction should be recorded separately.
Original hypothetical example, not an actual incident.
01Before work
- 1Report what was actually observed from a safe place.
- 2Prioritize evacuation and work stoppage if danger is imminent.
- 3Do not invent the cause of the incident or assign blame without evidence.
02During work
- 1Separate temporary access controls from a permanent fix.
- 2Assign an owner and clear verification criteria.
- 3Do not infer elimination of danger solely from before-and-after photos.
03Records and follow-up
- 1Original report, time and later amendments
- 2Immediate precautions, owner and completion report
- 3Reinspection outcome and unresolved issues
- 4Whether TBM and risk assessment were updated
