When records differ: original entry, dispute and correction
When work hours or safety acknowledgments are disputed, keep the initial statement distinct from the objection. Allowing a correction is different from preserving an auditable history.
Illustrative site situation
Illustrative situation: the worker recalls one full workday, but a foreman's draft shows half a day. Preserve both statements, supporting information and the time and reason for review.
Original hypothetical example, not an actual incident.
01Before work
- 1Identify the date, site and worker for each entry.
- 2Find out who recorded the original value and on what basis.
- 3Separate competing statements without silently deleting the original.
02During work
- 1Review the reason and evidence for the challenge.
- 2Distinguish acceptance, counterproposal and rejection, recording reasons.
- 3Keep the previous value, revised outcome and decision time.
03Records and follow-up
- 1Original entry and what it represented
- 2Worker objection and supporting records
- 3Reviewer decision and its reasoning
- 4Open, resolved or deferred status and later changes
